What's the difference between SIM free and unlocked?

When buying phones outright (not on contract) - what's the difference between SIM free and unlocked?

Unlocked might still have operator branding physically on the device and/or in the software, in the form of boot screens, apps and the like. But some don't. Unlocked often needs to be bought with a top up whereas sim free wouldn't.
