If your touchscreen is still responding: Menu => Settings => Privacy => Factory Data Reset.
When the screen is frozen:
Power off device or take out battery then reinsert it. To power off
press and hold power key until the menu pops up that offers flight-mode, silent profile or 'power off'. Take 'power of'.
Once the phone is shut down, press camera key + power key for about 10-12 sec (You will see Motorola logo then a yellow exclamation point in a white triangle.)
NOW: Press both (1st) Volume UP + (and then) camera key in this order for the Android System Recovery menu.
Use the volume key to scroll to 'wipe/factory reset'. Press the power button or the centre key of the D-pad to confirm. The phone should now reset.
