Is there just one iPhone 5 supporting all bands? I mean, there's not an LTE version and a non-LTE version?

2 types of phones - one for AT&T US and one for rest of the world - not compatible with each other.
Third version is for CDMA and will not be unlockable.

No, not 3g and 4g ones, but there are three different LTE versions

Yeah that's not quite right from what I've seen, the iphone 5 won't work on other uk networks 4g when it arrives. (possibly on 3 on the same band assuming they can shoehorn it into the small amount they've bought from ee)
