Do I need to manually check for HTC Desire Gingerbread on O2?

Or will the OTA update just happen without me doing anything?

I would not expect that O2 will pay for your upgrade. Probably would be manual checking or you might get a notification.
Either way upgrade over WiFi.

HTC's usually get a notification that an update is available. You can then decide to download there and then or opt to wait until you are on wifi before it starts (and thus avoid burning up your data allowance).
i.e. no need to keep checking
