The Milestone and Desire both run Android, but they also have Motoblur and HTC Sense. How does that work?

Sense and Motoblur are skinned user interfaces that are added on 'top' of the OS to improve usability and give the phone a unique look.
They are like themes you add to Windows - only they do a bit more than change the colours of toolbars. Menu structures are changed, shortcuts added etc etc
Think of the OS as a car's engine. The UI is the chassis and dashboard and gear stick and the upholstery...a theme would be just the new paint job.
We never actually see the OS (most of us) - what we would describe as the Windows 7 OS is actually just it's shiny user-friendly UI. You could do most things with just the run commandline window.
