If a phone gets replaced after, say, six months, do you start a new warranty period, or does it still start from the original date?

Varies from company to company. When I claimed something under warranty from Apple for instance, the repairs themselves had a 3 month warranty which ran parallel to the original warranty. So for example even if my warranty ran out for the whole device, if the repaired component broke again within 3 months, it'd be fixed.
